Overview

Summary:
 
The Clinical Access Referral and Engagement (CARE) Service Navigator plays a vital role in supporting Vinfen’s Community Behavioral Health Center (CBHC) Mobile Crisis Intervention (MCI) team and the CBHC Community Crisis Stabilization Coordinator by ensuring seamless follow-up care and connecting individuals to appropriate community and healthcare resources. This position ensures the integration of care through collaboration with internal and external partners and acts as the primary liaison with the Integrated Care Division for longer term care coordination. The CARE Service Navigator engages clients through assessment of their needs and helps make connections to the services needed.

Responsibilities

Job Duties and Responsibilities:
 
• Provides immediate, short-term follow-up support to individuals served by MCI and CCS to ensure continuity of care and connection to needed services and support.
• Provides education to clients regarding treatment options and provides referrals to appropriate programs.
• Coordinates and facilitates internal and external referrals, including warm hand-offs to community providers, partners and VBH Non CBHC Outpatient programs.
• Assists individuals in addressing unmet needs related to housing, food, transportation, healthcare, and other social determinants of health.
• Collaborates closely with the MCI and CCS coordinators to support integrated care planning.
• Serves as the primary point of contact between the acute services teams and the care coordination team to support coordinated and person-centered care.
• Informs clients, referral sources, and other community stakeholders about the Vinfen Community Behavioral Health Advisory Council and supports them in getting involved; recruits new members and participates in council meetings.
• Participates in regular CARE team meetings and Utilization Review meetings.
• Assists with networking and marketing in the community for the purpose of program expansion.
• Performs other related duties, as required.

Qualifications

Knowledge and Skills:
 
• Experience in behavioral health, case management, social work, or a related field preferred.
• Strong knowledge of community resources and systems of care.
• Excellent communication and organizational skills.
• Familiarity with EHR systems and commitment to accurate data entry.
• Ability to work collaboratively within a multidisciplinary team and across service systems.
• Knowledge of person-centered, strengths-based, recovery-oriented values and principles and modalities.
• Knowledge of substance use and psychiatric rehabilitation and recovery values, principles, and techniques.
• Knowledge of motivational interviewing, stage of change, and harm reduction techniques.
• Knowledge of trauma-informed and culturally responsive services.
• Sensitivity to cultural, religious, ethnic, disability, and gender issues.
• Ability to triage/balance competing priorities.
• Ability to make independent judgments and decisions.
• Ability to work in a professional and confidential capacity.
 
Typical Requirements:
 
Minimum of two years of experience working in human services or case management is preferred. Experience working with people living with Serious Mental Illness (SMI) and/or Substance Use Disorder (SUD) is strongly preferred. Preference is given to bilingual/bicultural applicants and those with personal lived experience of psychiatric conditions or substance use disorders. Access to reliable transportation to travel to BHAS sites as needed.
 
Preferred /Required Education:
 
BA/BS in human services-related field is preferred. In some instances, experience may be substituted for academic training.
 
Driving Requirements:
 
May be called on to drive for Vinfen using either a Vinfen van or personal vehicle. - A valid driver's license must be presented at the time of employment. Incumbents must be at least 21 years of age, have maintained a valid US driver's license for at least one year, and must be able to pass a driver's screening background check. If driving, must maintain a safe driving record, which is subject to annual checks. If using a personal vehicle, must possess and maintain adequate insurance.
 
Physical Effort:
 
Direct Care Roles - Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb as this is a very physically active position. Must be able to lift at least 25 pounds using proper lifting techniques or the use of a two-person lift. Ability to operate a computer and other office equipment such as a calculator, copier, and printer. Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways. Ability to assist clients with tasks of daily living. Ability to speak, hear, and communicate with clients, staff, and external representatives. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. For positions in day programs or group residences, the ability to assist in routine living activities including cleaning, meal preparation, vacuuming, shoveling, and grocery shopping.
 
Required Certifications/Trainings:
 
CPR within 2 weeks of hire, First Aid within 2 weeks of hire
