Overview

 

As a Per Diem Residential Nurse (RN) (Developmental Disabilities) you will participate in the comprehensive assessment of physical health of the people Vinfen serves on a per diem basis. Your work will be in clinically intensive residential programs tailored to meet the needs of an adult population who have brain injuries, intellectual and developmental disabilities in addition to complex health needs. In your role you will work together with the people we serve, their families, friends, employers, and other support staff to transform their lives.

As a Per Diem Residential Nurse (RN) you will function in a direct care nursing capacity. You will consult with other clinicians regarding health promotion and maintenance, medication administration and acute and chronic medical/ psychiatric crisis management for individuals served.

Responsibilities

Job Duties and Responsibilities:
 
The essential job duties/responsibilities of the position include but are not limited to the information listed below:
 
• Administer medication and medical treatment to persons served.
• Obtain and maintain required physician orders and other health related documentation as required by Vinfen, state, and federal guidelines.
• May collaborate with Vinfen Nurse Liaison for issues pertaining to the Medication Administration Program (MAP) (as described in MAP Consultant description in Vinfen MAP Policy & Procedure Manual.
• Provide education, consultation, and training to staff and persons served regarding changes in health status, health promotion and maintenance.
• Work as a liaison between program and community health care providers to promote access to community resources and quality urgent and routine medical/dental care.
• Attend and transport clients/persons served to medical appointments, as needed.
• Take and document client’s temperature, pulse, respirations, and blood pressure per IAP and as ordered by the primary care clinician.
• Collect sputum, urine and stool samples as appropriate and ordered by the primary care clinician.
• Administer medication and treatments as ordered by primary care provider.
• Maintain client’s/persons served medical equipment and supplies that includes, but is not limited to wheelchairs, walkers, Hoyer lifts, and hospital beds.
• Attend case conferences and attend internal and external clinical and divisional related meetings, as necessary.
• Provide staff education, consultation and training related to medical care management
for persons served during a pandemic, state of emergency or natural disaster.
• Adhere to all company and divisional and program specific policies and procedures, including safe work practices, worker’s compensation, universal precautions, and exposure control. Comply with accreditation and licensing standards, as well as regulations.
• Demonstrate best nursing practices and meet state requirement of continuing education to maintain RN licensure and certifications.
• Maintain current knowledge base of human service issues and trends as related to psychiatric, medical and rehabilitation issues.
• Provide direct nursing care to persons served, as needed.
• Support staff in crisis management, as needed.
• Perform other duties, as required.

Qualifications

 
  • A bachelor's degree in Nursing preferred, but experience may substitute for education. High school diploma or GED required.
  • Current R.N. license in state of service required.
  • 3-5 years nursing experience required.
  • Experience working with people with brain injury strongly preferred.
  • Strong time management and writing skills.
  • Must be at least 21 years of age and have a current, valid, US drivers' license for at least 6 months.
  • Must be able to become CPR, First Aid certified within 2 weeks of employment, Safety Care certified within 90 days of employment and MAP certified within 150 days of employment. Certification trainings are offered through Vinfen upon hire.
  • Must be able to successfully pass a CORI, reference, multi-state background and driving record check.
Typical Requirements:
 
Three to five years of experience working with people with psychiatric disabilities/developmental disabilities and medical illnesses and other co-occurring disorders preferred.
 
Preferred /Required Education:
 
A high school diploma or equivalent is required; Bachelor’s degree in Nursing is strongly preferred; Associate’s degree in Nursing is required.
 
Driving Requirements:
 
May be called on to drive for Vinfen using either a Vinfen van or personal vehicle. A valid driver’s license must be presented at the time of employment. Incumbents must be at least 21 years of age, have maintained a valid US driver’s license for at least one year, and must be able to pass a driver’s screening background check, which is subject to annual checks. If driving, must maintain a safe driving record. If using a personal vehicle, must possess and maintain adequate insurance.
 
Physical Effort:
 
Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb as this is a very physically active position. Must be able to lift at least 25 pounds using proper lifting techniques or the use of a two-person lift.  Ability to operate a computer and other office equipment such as a calculator, copier, and printer. Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways. Ability to assist clients with tasks of daily living. Ability to remain in a stationary position 50% of the time as needed. Ability to bend, reach, file, sit, stand, and move around the facility. Ability to speak, hear, and communicate with clients, staff, and external representatives. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
For positions in day programs or group residences, the ability to assist in routine living activities including cleaning, meal preparation, vacuuming, shoveling, and grocery shopping.
 
Required Certifications:
 
CPR is required within two weeks of hire
Safety Care is required within 90 days of hire
Required Credentials/Licenses:
RN in state of service required.
