Overview

Provide individual and group support to clients in a residential or day program/clubhouse. Teach skills to enhance independence and quality of life. Function in a professional role in order to fulfill programmatic needs/ routines.
 
Supervision and other duties may also be involved. 

Responsibilities

Job Duties and Responsibilities:

 

The essential job duties/responsibilities of the position include but are not limited to the information listed below:

 

• Assist in developing, implementing, and documenting individual treatment support plans.

• Responsible for a variety of daily tasks with clients. This includes rehabilitative interventions, behavioral interventions and monitoring, scheduling appointments, monitoring progress, providing transportation, and other related tasks.

• Provide role model and support to clients. Assist them in becoming fully integrated into their communities.

• Assess clients’ physical and emotional status, and report changes to supervisor.

• Ensure program site safety and maintenance as necessary at all times. Oversee and participate in the daily living and cleaning of the program, inside and outside with clients.

• Comply with all corporate and divisional policies and procedures, including safe work practices, worker’s compensation, universal precautions, and exposure control.

• Complete various paperwork and logs including communication logs, community membership logs, chore schedules, time sheets and application forms.

• Perform crisis intervention, as needed.

• Perform other related duties, as required

Qualifications

Bachelor's degree preferred. High School diploma or GED required
 
Knowledge and Skills:
 
• Knowledge and use of advocacy techniques
• Knowledge and use of different communication styles
• Knowledge of empowerment and self-advocacy techniques
• Knowledge of required physical personal management skills
• Knowledge of crisis intervention techniques

Pay Range

USD $20.97 - USD $20.97 /Hr.
