Summary:
The Clinical Supervisor (CS) conducts intake evaluations, functional, and clinical assessments and develops Community Support Plans (CSP) and crisis plans for all Respite consumers. The CS provides clinical supervision to Respite staff, facilitates clinical groups and implements individual clinical interventions as indicated on the person served’s CSP. The CS is a key member of a larger Respite team and works closely with Community Health Workers, Certified Peer Specialists, Nurse, and Mental Health Counselors under the supervision of a Program Director
Job Duties and Responsibilities:
• Provides outreach to and engages people referred to the Respite program via phone, video and in person within various community settings to promote effective service delivery and use.
• Completes required clinical, social, and functional assessments.
• Conducts initial and ongoing risk assessment and designs personal crisis management plans with members, which include relapse prevention and harm reduction strategies and reviews and approves crisis management plans designed by non-licensed team staff.
• Develops and implements person-centered Community Support Plan (CSP), crisis plan and discharge plan.
• Trains and supports staff in delivering clinical evidence-based practices and best practices to all persons served (PS).
• Oversees service delivery to all assigned PS, including participation in services, treatment and support skills planning and implementation of recovery goals, peer and family support, readiness for discharge, and support through service and life transitions, including acute care and housing transitions.
• Collaborates with members, Vinfen staff, health care and inpatient providers to coordinate safe discharges following planned or unplanned admissions to a medical, substance use or psychiatric acute care facility or ED (including hospitals, Rehab facilities, and substance abuse programs).
• Addresses/resolves system problems impeding diagnostic or treatment progress with members; proactively identify and resolve system delays and obstacles to coordinate care. • Meets directly with members/family to assess needs and develop an individualized ongoing care plan in collaboration with physicians and the multidisciplinary team to support transitions and CSP goals.
• Initiates referrals to transitional services which may include but are not limited to home health care, hospice, partial hospitalization, substance abuse and/or outpatient behavioral health treatment, or medical equipment and supplies.
• Refers members to needed community or social service organizations to address barriers to health and community tenure and support members to use those services and resources effectively.
• Provides members health and wellness coaching to promote the development of healthy behavioral change.
• Documents as required in the electronic medical record.
• Performs other duties, as assigned.
Physical Effort:
Direct Care Roles - Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb as this is a very physically active position. Must be able to lift at least 25 pounds using proper lifting techniques or the use of a two-person lift. Ability to operate a computer and other office equipment such as a calculator, copier, and printer. Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways. Ability to assist clients with tasks of daily living. Ability to speak, hear, and communicate with clients, staff, and external representatives. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. For positions in day programs or group residences, the ability to assist in routine living activities including cleaning, meal preparation, vacuuming, shoveling, and grocery shopping.

Requirements:
• Minimum of 3 years clinical and case management experience preferred.
• Clinical licensure at the independent level.
• Experience working with people living with SMI and/or SUD.
• Preference given to bi-lingual/bi-cultural applicants and those with lived experience of psychiatric conditions or substance use disorders.
Required Education:
Master's degree in Social Work, Mental Health Counseling or related field required.
Driving Requirements:
Driving is a requirement for this position using either a Vinfen van or personal vehicle. - If using a personal vehicle, you must possess and maintain adequate insurance as well as maintain a safe driving record which is subject to annual checks. A valid driver's license must be presented at the time of employment. Incumbents must be at least 21 years of age, have maintained a valid US driver's license for at least one year, and must be able to pass a driver's screening background check.

CPR within 2 weeks of hire, First Aid within 2 weeks of hire, MAP within 150 days of hire
