The essential job duties/responsibilities of the position include but are not limited to the information listed below:
• Functions in role of Medication Administration Program Consultant (as described in MAP Consultant description in Vinfen MAP Policy & Procedure Manual) to assigned sites.
• Trains staff to receive MAP Certification.
• Monitors program adherence to medication administration policies, procedures, and regulations.
• Reviews medication policies and occurrence reports.
• Performs regular reviews at site(s) regarding implementation of MAP.
• Retrains staff in MAP procedures, as needed and documents accordingly.
• Serves as liaison to EOHHS/DPH and funding agency MAP Coordinators.
• Provides 24/7 on-call nursing coverage to assigned sites on a rotating schedule.
• Completes Vinfen Health Assessment of persons served at intake and as needed with regard to medical and psychiatric conditions.
• Provides relevant education and training to program staff regarding medical and psychiatric issues for new persons served to provide successful admission to Vinfen program.
• Provides education, consultation, and training to staff and persons served regarding changes in health status, health promotion, and maintenance.
• Identifies, develops, and enhances relationships with community resources for persons served.
• Works as a liaison between Vinfen and community health care providers to promote access to community resources and quality urgent and routine medical/dental care.
• Attends case conferences and internal and external clinical and divisional related meetings, as necessary.
• Contributes to the development of support/treatment plans for person’s served for medical and psychiatric issues.
• Provides staff education, consultation, and training related to medical care management for persons served during a pandemic, state of emergency, or natural disaster.
• Demonstrates best nursing practice and meets state requirement of continuing educational to maintain RN licensure and certifications.
• Maintains current knowledge base of human service issues and trends as related to psychiatric, medical, and rehabilitation issues.
• Provides direct nursing care to persons served, as needed.
• Provides clinical supervision to LPNs, as necessary or assigned.
• Perform other related duties, as required.
