Overview

The Transition to Independence (TIP) program provides coaching and facilitation to transitional age youth and young adults (YYA) in meeting their life goals. The TIP TF provides support for 10-12 YYAs using mentoring, teaching skills, contingency contracting, counseling, and problem solving to maximize the likelihood ofsuccess for each YYA served. The TF develops and monitors in partnership with the YYA a person-centered plan and goal setting process. The Facilitator helps each YYA recognize that life situations provide both successes and challenges that provide the YYA with valuable experiences and opportunities to learn. The TF assists the YYA in identifying
that the integration of family members, friends or school personnel (for YYA under 18 or in college); and other relevant natural supports are powerful linkages in this process. The Senior Facilitator is a Certified TIP Trainer and provides training and support to TIP Facilitators in the areas of Strengths Discovery Assessment, the TIP Core Competency Action Plan, and the TIP Solutions Review process, as well as other areas defined by the Program Director related to service provision,
assessment, planning, and TIP core competencies that enhance the YYAs likelihood of reaching
goals in relevant life domains.

 

Job Duties and Reprehensibility

The essential job duties/responsibilities of the position include but are not limited to the information listed below:

  • Provides training to TIP Facilitators in the areas of the Strength Discovery Assessment process, engaging YYAs, developing TIP Core Competency Plans, and developing implementation strategies utilizing TIP modalities- as directed by YYA voice
  • Acts as a resources for the TIP Facilitators regarding educational, social, parenting, and housing opportunities within the community
  • Helps TIP Facilitators in developing TIP Solutions Review Documentation and preparing to present at TIP Solution Reviews
  • Accompanies TIP Facilitators on initial assessment visits for the purpose of supporting and modeling for each new Facilitator in correct use of modalities and engagement strategies
  • Delivers evidence informed/based practices, interventions and modalities, as required by the service
  • Interfaces with adult mental health case managers to ensure continuity of age-appropriate services for individuals as they approach 18 years of age and older
  • Assists the YYA in the identification of obstacles to living independently, obtaining employment, obtaining post-secondary education, and in developing supportive social and recreational activities
  • Refers YYA to appropriate clinical, medical, and behavioral specialists as requested, helps YYA in acquiring access to community settings and resources (e.g. employment, housing, social security benefits)
  • Performs other duties as assigned
