The Youth Community Crisis Stabilization (YCCS) Licensed Practical Nurse provides direct service and daily nursing interventions in a YCCS program for youth who have behavioral health symptoms. The Youth Community Crisis Stabilization program is designed to meet the needs of individuals ages 13-18 who have behavioral health symptoms that require 24-hour per day care in an unlocked treatment setting. The LPN provides nursing and medication administration services, health promotion training and access to medical care to clients. The LPN functions as part of an interdisciplinary team and support individual's treatment goals using psychoeducation, crisis prevention planning and skills training. The LPN's efforts ensure that the YCCS program meet high quality standards and provide value to individuals and a community care system.


Additional responsibilities include but at not limited to:



  • Assist the YCCS RN with filling prescriber's orders, administering medications, and monitoring vital signs.

  • Work with the YCCS Direct Service Workers in ensuring an environment that promotes safety, recovery, and treatment.

  • Contribute to the assessment, treatment plan, individualized crisis planning, discharge planning and care coordination for each individual.

  • Implement treatment plans through use of recovery-oriented, motivational interviewing, harm-reduction, trauma-informed, culturally competent and strengths-based rehabilitation approaches to help youth successfully and safely transition to home or an alternate optimal setting.

  • Provide nursing care, teaching and assistance with activities of daily living, according to each client's plan.

  • Take and document individual's vital signs, including temperature, pulse, respirations, and blood pressure per prescriber's orders. May include pulse oximetry.

  • Conduct wellness psycho-educational groups for individuals.

  • Assist in the liaison between individuals and community health care providers to promote access to community resources and quality care at all levels.

Qualifications


  • A high school diploma or equivalent is required. Graduate of accredited LPN program required.

  • Licensure as a Licensed Practical Nurse (LPN) in the state of Massachusetts required

  • Three to five years supervised practical experience, including internships, in a clinical or rehabilitation setting with persons with psychiatric conditions preferred.

  • Prior experience working with youth and families using evidenced based practices in a community mental health and/or substance use clinic, residential, acute or sub-acute or similar setting desired.

  • Preference given to bi-lingual/bi-cultural applicants and applicants with lived experience of psychiatric conditions.

  • Comfortable using a computer and experienced working in an electronic health record, proficient in communicating effectively, both written and verbally, with varying audiences

  • Familiar with MassHealth and managed care practices

  • Must be able to become CPR certified within specificed timelines. Vinfen offers training upon hire.

  • Must be able to successfully pass a multistate CORI, SORI, reference check, employment verification, education and credentialing background check.

Pay Range

USD $37.50 - USD $37.50 /Yr.
